Welcome to HolistiCal Primary Care and Advanced Nursing Services, Inc. (“HolistiCal,” “we,” “our,” or “us”). By booking an appointment, receiving care, or using our website or telehealth services, you agree to the following Terms and Conditions. Please review them carefully before proceeding.
1. Appointments & Services

Services are provided by a Board-Certified Family Nurse Practitioner (FNP-BC).

NP supervised by Dr. Boris Vaisman, MD.

Telehealth services are available only to patients physically located in California at the time of the visit.

Appointment times are subject to provider availability. Emergencies and urgent care needs may receive priority scheduling.

For missed or late appointments, we reserve the right to charge a no-show or late cancellation fee as outlined in our clinic policies.
2. Patient Responsibilities
To ensure the best possible care, you agree to:

Provide accurate, complete medical history and contact information.

Notify us promptly of any medication changes, allergies, or new symptoms.

Follow treatment plans and attend recommended follow-up visits.

Use emergency services (911 or nearest ER) when advised or in case of urgent/life-threatening situations.
3. Payment & Insurance

We accept Medicare, Aetna, Multiplan, UnitedHealthcare, and other contracted insurances. Medi-Cal contracts are pending.

Patients are responsible for co-pays, deductibles, and any non-covered services at the time of visit.

For self-pay patients, payment is due at the time of service.

We accept major credit/debit cards and HSA/FSA cards.
4. Refunds

Payments for completed visits are non-refundable.

If a billing error occurs, please contact our office within 15 days, and we will review and issue an adjustment or refund if appropriate.

Prepaid packages or services may be refundable on a prorated basis if unused, subject to administrative fees.
5. Product Sales (Supplements, Health Kits, etc.)

Any supplements or wellness products purchased from our clinic are eligible for return within 15 days if unopened and in original condition.

Opened products or items used by the patient cannot be returned for safety reasons.
6. Telehealth Consent
By scheduling or attending a telehealth visit, you must be physically located in California, USA and you acknowledge and consent to:

Receiving medical care via secure video or phone platform.

Understanding that telehealth has limitations and may not be appropriate for all conditions.

Following up in person or at the emergency department if advised by the provider.
7. Privacy & Confidentiality
Your health information is protected under HIPAA (Health Insurance Portability and Accountability Act). We maintain strict confidentiality and will not share your data without your consent except where required by law.
8. Modifications
We reserve the right to update these Terms & Conditions, our clinic policies, and fees at any time. Updates will be posted in our office and/or on our website, with the effective date clearly indicated.
9. Disclaimer & Limitation of Liability
Our services are provided “as is” and without guarantee of specific outcomes. We are not liable for indirect, incidental, or consequential damages related to your use of our services. You are responsible for following medical advice, taking medications as prescribed, and seeking emergency care when necessary.
